The following forms are also required in order to process your application: REGISTRATION/INSTRUCTIONAL FEE: A non-refundable fee of $125 per student will also need to be paid with this form. A 3% convenience fee will be charged when using your credit card when paying this fee. STATE OF OHIO MEDICAL FORMS: THESE FORMS MUST BE ON THE ACTUAL STATE OF OHIO FORMS. These 2 forms have been uploaded and attached later in this registration. EACH FORM MUST BE PRINTED, COMPLETED AND RETURNED TO THE LEARNING LOFT PRESCHOOL OFFICE FOR EACH CHILD BEING REGISTERED in order for your registration to be complete. The State of Ohio does not allow us to create these on-line nor do they have a fillable version for us to include. Because the medical form has to be signed by your child's doctor, you have until August 15, 2022 to return this form to the Learning Loft Office and your registration spot to be held.
